What is MIPS?
The Quality Payment Program (QPP) is a portion of MACRA, which replaced and consolidated the Physician Quality Reporting System (PQRS), Medicare Electronic Health Record (EHR) Incentive Program (Meaningful Use) and Value-based Payment Modifier (VM) programs to create the Merit-based Incentive Payment System (MIPS).
Eligible clinicians can earn up to a total of MIPS composite performance score (0 – 100) across four performance categories, though each category is weighted differently, and the weight may change year to year. The CPS values are ranked from highest to lowest, and the relative ranking of each score determines how provider payments are adjusted, dictating whether a bonus or penalty results as well as its size.

How much can MIPS adjust payments?
Steadily increasing penalties and incentives under MIPS can have a large impact on your bottom line.
In 2021, by not participating in the Quality Payment Program or obtaining a low MIPS final score your practice will incur the -9% maximum penalty payment adjustment. Achieving a moderate MIPS final score will allow for penalty avoidance, and may provide a slight incentive. Falling into the exceptional performance range will result in a +9% payment adjustment, and possibly an additional performance bonus up to 10%.
Over the long term, MIPS reporting has the potential to affect a provider’s public reputation. These publicly-published results could influence patients’ choice of physicians (directly, or via insurers or employers that steer them to the highest performing doctors).
